- Copays are due at the time of service
- 1/2 payment is required for all medical visits
- Payment in full is required before glasses/contacts are ordered

- Driver license or state ID
- Credit card on file
We are in-network and able to to see PPO/OAP type of insurances. If we are not in-network with your insurance we provide private pay discounts and help with submitting your your insurance for out of pocket benefits. Any questions - just text or call!
